Government-Funded Home Care Through Family-Managed Home Care

At NurseFirst Home Healthcare, we proudly support families participating in Ontario’s Family-Managed Home Care program by delivering compassionate, professional care in the comfort of your home. 

The Family-Managed Home Care program is managed by Ontario Health at Home (OHAH) and  provides eligible individuals or their substitute decision-makers with government funding to arrange their own home care services. Rather than having a provider assigned, families have the freedom to choose their preferred healthcare professionals, determine when services are delivered, and oversee their care based on their unique needs.

To qualify, patients must first be assessed by Ontario Health at Home, meet the program’s eligibility requirements, and have an approved plan of care. The program is available to eligible individuals, including children with complex medical needs, adults with acquired brain injuries, home-schooled children requiring care, and others with exceptional healthcare needs.

Families participating in the program are responsible for coordinating care, hiring providers, and managing funding and reporting requirements. Directing your care through Ontario Health at Home-Government Funded Program

STEP 01: Ontario Health at Home

Contact your Ontario Health at Home care coordinator to see if you qualify for the program called Family- Managed Home Care

STEP 02: Pass requirements

Family-Managed Home Care is designed for eligible patients in the following groups:

  • Children with complex medical needs 

  • Adults with acquired brain injuries 

  • Eligible home-schooled children 

  • Patients in extraordinary circumstances, as assessed by Ontario Health atHome

STEP 03: Receive Your Funding

After Ontario Health at Home approves your funding, the funds are provided directly to you or your substitute decision-maker. This gives ou the flexibility to select the home healthcare provider you trust, including NurseFirst Home Healthcare.

Step 1: Check Eligibility

Contact your Ontario Health at Home care coordinator to see if you qualify for the program called Family-Managed Home Care.

Step 2: Review Requirements

Family-Managed Home Care is designed for children with complex needs, adults with brain injuries, home-schooled children, and exceptional cases.

Step 3: Receive Funding

After Ontario Health at Home approves your funding, the funds are provided directly to you or your substitute decision-maker.

Step 4: Contact NurseFirst Home Healthcare

After funding is confirmed, check with NurseFirst Home Healthcare for availability.
